PROGRAMS WE OFFER

Program Eligibility:

1. Recipients must be a resident of South Carolina and have not received an EUAP award in the past 12 months of the last approval.
2. An individual diagnosed with breast cancer that is actively in treatment at the time of the application process.  
3. An individual diagnosed with breast cancer that has completed treatment in the last year.  Please note: ongoing maintenance treatment does not qualify as active (i.e. Oral Chemotherapy pills, Immunotherapy, Reconstruction, oral hormonal therapies and future scheduled surgery or treatments.)
4. An individual that does not have a diagnosis of breast cancer, is uninsured or underinsured, and is in need of breast screening services.

Referral of Recipients:

1. Individuals can self-refer, by completing an EUAP application.  The application must be submitted via email or mail to the Embracing U Organization. 
2. Referrals from a healthcare provider or representative must be submitted with a written request describing hardship and/or need.

The amount funded can be equal to the maximum amount designated for program funding ($350.00) or less as deemed necessary based on review by the Board of Directors.
